General requirements

The questionnaire can be filled out on a computer (after which you need to print it on an A4 sheet) or by hand. If you have filled out the document completely, you can sign it immediately.

1. Enter your last name in Latin, as in the foreign ID, and duplicate it in Russian through a slash (RUPKIN/RUPKIN).
2. If after marriage a woman left her old surname, then “No” should be marked in the corresponding box. Otherwise, the column is filled in similarly to the first paragraph. If the surname has been changed several times, then they should be entered in the chronology, even if the existing surname is the same as the original one.
3. The column where you need to enter your name is filled out in the same way as the column for your last name. If there has been no name change, then information from the certificate confirming the fact of birth is indicated.
4. date of birth format – year-month-day.
5. Enter the city or region, region where you were born. If your hometown had a different name at the time of birth, that name should also be entered. Exact information can be found in the certificate. about birth.
6. If your birth took place before the collapse of the USSR, before 19991 inclusive, then indicate “USSR”, after this period - “Russia”.
7. Citizenship at birth – “RF” or “USSR”, today – “Russian Federation”.
8. Indicate your marital status.

9. Enter . Signs No., N, # are prohibited. Separate series and number with a space. Be sure to indicate the validity period of the certificate, the date of its issue, and by whom it was issued.

10. Indicate your actual place of residence (that is, your actual address). If it differs from what is indicated in the internal document, you may be required to provide duplicates of the apartment rental agreement or a certificate from work or study.
11. Please provide a current email address.
12. Enter your mobile number. After submitting the papers, you should be in touch, as officials may need to clarify something. If available, house. phone number, please indicate that too.
13. Designate your prof. current activities (pediatrician, teacher, accountant
). Indicate “Pensioner” if this applies to you. If you are studying – “Schoolchild”, “Student”. If you are a housewife – “Housewife”. Field about prof. activities must be completed!
14. Indicate information about the place of work or training - address, telephone number, name. If you do neither one nor the other, write “Not applicable.” Abbreviations, streets, JSC, JSC, company name are not translated. Indicate in transliteration.


15. Write the country of destination and purpose of travel.
16. If, for example, the first country is Germany, then write it. If you plan to transfer in the Schengen zone, for example, in Riga, then indicate “Latvia”. If you are traveling by car and will be passing through several states during your trip, list them in this field.
17. Indicate the number of entries you are requesting (single, double or multiple). The latter option requires purchasing insurance.
18. Enter the date of arrival (entry), as well as departure (departure) from the Schengen zone.
19. Write how many days you are going to stay in the Schengen zone. Count from the moment you cross the border. If you drive a personal car, then take this indicator into account.
20. If you have been issued Schengen visas over the last 3 years, then enter their validity periods. If you are requesting a visa permit for the first time or the visa was issued to you more than three years ago, then write “No” in the field.

21. If the tour is planned by invitation, then you should provide information about the person who invited you (initials, email, telephone). If there is no invitation, please indicate your telephone number, fact, address, and the name of the hotel where you plan to live. Confirmation of your reservation will help you fill it out.

22. It is necessary to decide on the method of financing the trip, as well as the responsibilities of each party, if there is an invitation.
23. Enter a list of documents that you will attach to the application and questionnaire.
24. Indicate whether you have previously
